(Cth) and the applicable Health Privacy Principles in the Health Records Act 2001 (ACT) outline

complies with these principles and explains how Conny Koenderink Counselling applies them

Conny Koenderink Counselling collects personal and health information to assess and support
clients through counselling and/or group therapy. Data collection depends on the type of
service you are accessing and our individual agreement regarding services.
This information may include your full name, date of birth, address, telephone number, email
address, billing and appointment information, general wellbeing information and other relevant
personal information.
Conny Koenderink Counselling collects personal information through a variety of means,

- Our intake form

- The guardian of those under 18 years of age
- Referral forms from your GP or other service or third party.

Conny Koenderink Counselling does not retain any credit card or banking details once
processing of payment is finalised.

From time to time, Conny Koenderink Counselling may make changes to this privacy policy.
